Laska Dental is going paperless for the safety of our customers and our staff. In order to better serve you, we are asking that your paperwork be filled out before your appointment. Please do not put any sensitive information, such as date of birth, social security numbers, or driver's license numbers, on these forms. We will gather this when you come into our office.
